65 RADIOLOGIC TECHNOLOGY September/October 2006, Vol. 78/No. 1 Knee pain is a common reason for patient visits to a physician office. Cluette suggests common causes of knee pain, including arthritis, ligament injuries, cartilage injuries, meniscal tears, patellar tendonitis, chondromalacia patella, Bakers cyst, bursitis, Osgood-Schlatter disease, osteochondritis dissecans and gout. Certainly there are other causes; the job of imaging professionals is to facilitate the diagnosis of such ailments. In the past, plain film radiography was used as the first imaging modality to aid the evaluation of the painful knee; however, today plain films are used primarily to evaluate joint space narrowing and for major trauma events. Positions for these cases include the traditional posteroanterior (PA) projections and lateral positions. Additionally, joint space narrowing is demonstrated by weight-bearing knee studies such as the Rosenberg method. Because of the many soft tissue structures within the knee, it is impossible to adequately evaluate the knee joint with plain radiographs. In fact, “[O]ver the past 15 years, magnetic resonance (MR) imaging has become the premier, first-line imaging study that should be performed in the evaluation of the painful knee.” This article describes some of the soft tissue components of the knee joint. Injury occurs when these structures become damaged, usually with resultant tendon, cartilage or ligament tears. Traumatic injury can be a major cause of knee problems, but damage can also be attributed to physiological processes such as gene mutations. Furthermore, this article defines internal derangement and concludes with an example of a torn meniscus demonstrated by the use of MR imaging.

The biological concept of "internal derangement and osteoarthrosis": a diagnostic approach in patients with temporomandibular joint pain?

OBJECTIVES We sought to investigate whether the finding of temporomandibular joint (TMJ)-related pain may be linked to magnetic resonance (MR) imaging findings of TMJ internal derangement and TMJ osteoarthrosis. STUDY DESIGN The study consisted of 194 consecutive TMJ patients. Criteria for including a patient with a painful TMJ were as follow: report of orofacial pain in the TMJ, with the pre...

The accuracy of sonography for evaluation of internal derangement of the temporomandibular joint in asymptomatic elementary school children: comparison with MR and CT.

BACKGROUND AND PURPOSE In order to clarify the incidence and evolution of disk displacement in the temporomandibular joint (TMJ) in children, we performed a longitudinal analysis in 18 subjects. Some investigators have suggested that sonography can provide information about the articular disk position of the TMJ. The purpose of this study was to determine the diagnostic accuracy of sonography f...

Clinical, MRI and arthroscopic correlation in internal derangement of knee.

BACKGROUND The traumatic or degenerative internal derangement of the knee requires certain investigations for the establishment of diagnosis, in addition to clinical history and a thorough physical examination. The use of arthrography and arthroscopy improves the accuracy of the diagnosis. MRI scanning of the knee joint has often been regarded as the noninvasive alternative to diagnostic arthro...

MR of osteochondritis dissecans and avascular necrosis of the mandibular condyle.

We studied 40 patients exhibiting radiologic changes of either osteochondritis dissecans (OCD) or avascular necrosis (AVN) involving the mandibular condyle to evaluate the structural changes associated with these lesions when using high-field-strength MR imaging. Various clinical indications for imaging each patient with routine radiography, tomography, and surface-coil MR included headache, te...

Diagnostic imaging of the temporomandibular joint: recommendations for use of the various techniques.

The indications for an imaging study of the TMJ are the same for MR, CT, and arthrognaphy; only those for a plain film study differ. In general, an examination is warranted (1) when the diagnosis of an internal derangement is in doubt and must be established before therapy can be instituted; (2) if the diagnosis is not in doubt but confirmation or documentation is required, such as for third-pa...

Normal MR imaging anatomy of the knee.

Magnetic resonance (MR) imaging is the preferred imaging modality for evaluating internal derangement of the knee, due to its superior soft tissue contrast resolution, multiplanar imaging capability, and lack of ionizing radiation. The superb image quality facilitates learning of normal imaging anatomy and conceptualizing spatial relationships of anatomic structures, leading to improved underst...
